Immunization Requirements 

Families of  TVHS Juniors-

The Pennsylvania School Health Act stipulates that all children, upon original entry into school and again in 6th and 11th grades, shall receive a physical examination.

You are encouraged to have the examination performed by your family physician since booster immunizations may be given at this time. Your child’s doctor needs to complete the “Private Physician’s Report” form at the time of the examination.

In accordance with the NEW immunization requirements from the Pennsylvania Department of Health and Pennsylvania State Board of Education a second meningococcal A immunization is required to enter grade 12. Your physician would most likely be administering this second meningococcal immunization at your child’s grade 11 physical examination. Without this immunization your child will not be allowed to enter school for grade 12.  Click here for more information about immunizations. 

For your convenience, the physical form can be accessed here because your child may be seeing the doctor for another reason, such as driver’s permit, work papers, or camp physicals.  Examinations performed after August 27, 2019 (the class of 2022’s first day of 10th grade) will be accepted for the 11th grade physical requirement.

 Please return the completed form to the nurse's office as soon as your child’s physical examination is performed.

Please make a copy of this for your personal record. Also for confidentiality, please place it in an envelope and label it “High School Nurse.” Please return this physical examination form to the nurse any time before March 2, 2021.
